Survey Materials

Below are the survey materials that were sent to you for your participation in the Household Travel Survey. 

First is the Household Questionnaire, which you received with the original letter.  If you need another copy, you can view and print it out here.

The Household Diaries, for each person in your household, were mailed to you after you spoke with the NuStats representative and set up a travel day for your household.  If you need another copy (or copies) of your Diary, you can either call 1-877-261-4621 or view and print out a copy here.

Also available are examples of individual travel days.  Please refer to these for further information on how to fill out your travel diary.
